Vaccine labels: when a label can mean the difference between life and death

Among all label applications, vaccine labels and labels for the cold chain supply chain perhaps carry the greatest responsibility. This is no longer a matter of aesthetics or brand recognition—this is a public health safety issue. A label that peels off during vaccine transportation at -70°C can cause a vial to lose its identifying information, leading to confusion, waste, or worse, administering the wrong vaccine to a patient.

The vaccine cold chain is one of the harshest environments any label can face. Temperatures can range from 2–8°C for most standard vaccines, down to -20°C or even -70°C for mRNA vaccines and specialty biologics. At these temperatures, conventional adhesives turn brittle, lose their grip, and peel away from glass vials or syringes. Printing inks can crack or fade. Protective laminate layers can shrink and cause the label to curl.

Specific Technical Requirements for Cold Chain Labels

Labels for vaccines must meet a combination of stringent standards simultaneously. First, the special freezer-grade adhesive must maintain bonding strength at extremely low temperatures while remaining flexible enough to avoid cracking when the glass vial contracts. Second, the label material—typically BOPP or PET film—must have a coefficient of thermal expansion compatible with glass to prevent shear stress from causing label delamination. Third, all information printed on the label, including the vaccine name, lot number, expiry date, GS1 barcode, and warnings, must remain clearly legible throughout the entire cold chain journey, even when the vial surface is subject to condensation during the thawing stage.

In particular, pharmaceutical labels for vaccines must also meet serialization and traceability requirements. Each vaccine vial carries a unique identifier (typically a GS1 DataMatrix) that enables tracking of that vial from the manufacturing plant to the point of administration. This system demands absolute accuracy in printed data, zero duplication, and codes that are 100% scannable by handheld scanners at vaccination sites—where lighting and environmental conditions are not always ideal.

Tamper-evident and error control in vaccine label production

Tamper-evident labels are a mandatory requirement for many types of vaccines. These labels are engineered to leave irreversible traces when peeled off, preventing vial caps from being opened and vaccines from being replaced with counterfeits. When peeled, the label either fractures into small fragments or leaves a patterned adhesive residue on the vial cap, making reapplication impossible.
